I was offered Sandrena gel as an alternative to Oestrogel sachets and I find it easier to use than pump dispensers. The effects were noticeable within about 4 weeks — my sleep improved first, then my hot flushes settled. I use it on my thigh every morning and it's become as routine as cleaning my teeth.

The transdermal route bypasses first-pass hepatic metabolism, which appears relevant to tolerability. Dose titration is also more straightforward with patches or gel, as the response can be assessed gradually. For the progesterone component, micronised progesterone tends to be better tolerated than older synthetic progestogens, with fewer reports of mood disturbance. Individualising both the route and formulation seems to be key to adherence.
