Cooling bedding and a small personal fan made a real practical difference to the night sweats even though they didn't treat the underlying issue. An important clinical distinction I make with my patients is whether their sleep disruption is primarily vasomotor — night sweats waking them — or primarily insomnia — difficulty initiating or maintaining sleep without obvious temperature triggers. Night sweat-driven sleep disruption responds well to oestrogen replacement. Pure insomnia without vasomotor symptoms may respond better to progesterone, CBT-I, or a combination. Many women have both components, which is why a thorough sleep history matters. I see too many patients treated with hypnotics when the underlying issue is hormonal.
