Tried
Estradiol Gel helped somewhat with my difficulty concentrating at work — energy improved, though the brain fog lingered. Not a miracle but not nothing either.

I told my GP I couldn't find words, kept forgetting appointments and felt like I was losing my mind. She told me it was just stress and anxiety and offered me sertraline. I'm a nurse — I know what anxiety feels like and this wasn't it. It took me six months and a private consultation to get HRT. By then I'd started questioning my ability to do my job safely. The brain fog was cognitive impairment, not stress, and it improved substantially once I had adequate oestrogen. The dismissal by my GP was the most damaging part of the whole experience.

Dr Louise Newson has emphasised that testosterone plays an underappreciated role in female cognitive function. Some women report that oestrogen alone does not fully resolve brain fog, and the addition of testosterone — typically at physiological female doses — provides the remaining cognitive benefit. In clinical observation this pattern is more common in postmenopausal women who have been oestrogen-depleted for longer periods. Testosterone influences dopaminergic pathways which affect motivation, drive and executive function — symptoms that oestrogen may not fully address.
