Why it gets missed
Heavier and less predictable bleeding is a common part of perimenopause, so it gets accepted as normal. Iron loss over months of it is also common, and produces exactly the kind of flat, breathless tiredness that gets put down to being busy.
The two are rarely mentioned in the same appointment, because they arrive as separate complaints.
What is worth mentioning together
How heavy, in plain terms. Flooding, needing to change protection hourly, or bleeding through at night are all worth saying out loud rather than editing down.
How long it has been like this.
The tiredness, in the same breath, so the two are looked at as one picture.
Very heavy bleeding, bleeding between periods, or bleeding after sex are all reasons to see a doctor rather than wait, whatever else is going on.
Energy is the thing every other change in these years is paid for out of. Protecting it is what makes the rest possible.
